If the documents requested refer to you personally, you must provide proof of identification (e.g. a scan of your driver's licence or passport). Without this proof of identity an agency is unlikely to grant access to personal information.

 

FOI Online Portal - Collection Notice

The infrastructure for the Freedom of Information (FOI) Online request portal is managed by the Department of Justice and Community Safety (DJCS) with oversight from the Department of Premier and Cabinet (DPC).

DJCS and DPC handle your personal information in accordance with the Privacy and Data Protection Act 2014 (Vic) (PDP Act), Public Records Act 1973 (Vic) and other applicable legislation.

Collection of personal information

DJCS is collecting your information for the purpose of allowing you to submit your FOI request online to the agency selected by you. Your personal information will therefore be shared with the specific agency to which you are submitting your request.

Westpac Banking Corporation (ABN 33 007 457 141) may also be provided with your personal information in order to process the payment of your application fee.

Consequences of not providing your personal information

If you do not provide your personal information as part of your FOI request submission, then it may not be possible for an agency to process your FOI request. Agencies may need to contact you to discuss the terms of your request and/or to provide you with the documents that you have requested.

If you are requesting access to documents as part of an FOI request that contain your personal information, then you are likely to be required to provide proof of identity with your application (e.g. a copy of your Australian Drivers Licence or passport). Without proof of identity an agency is unlikely to grant access to personal information.

Access and correction

You are entitled to access and correction of your personal information. If you wish to attach any additional documents to support your application, for example, an authority from a third party to access their personal information, they must:

  • be no larger than 5MB each
  • have a combined size of all files not larger than 5MB
  • be a maximum of 3 files
